We at Sunrise Wellness Care, a Medi-Cal, CalAIM provider, want to make you aware of additional services available to you as a Medi-Cal beneficiary. We strongly feel you will benefit from the personalized care we can offer you at the Sunrise Wellness Care program.
Pending qualification, these services are available at NO COST to you!
What is the Sunrise Wellness Care Program?
- Individualized plan of care and treatment for medical, behavioral, social and cultural needs
- Comprehensive care coordination
- Health education, in both one-on-one and group settings
- Community based and family support system

> Personal health coaches
Your personal health coach will be available to you in person or over the telephone. He/she will work with you and your health care providers to develop a personalized health care plan that meets your specific needs. They will be there for you and help you navigate the healthcare system and achieve your goals.
> Social and community resource counselors
Our team of resource counselors are there to assess your physical needs and facilitate access to resources that will support a better quality of life, things like; food and clothing, shelter and housing, arrange transportation to medical appointments and other basic needs.
> Behavioral and mental health resourcing
Our team will work with you to facilitate your access to behavioral and mental health professionals
