The state of California (DHCS) and your Medi-Cal plan have established certain criteria; you must be enrolled in a Medi-Cal managed care health plan, have certain chronic and/or mental health conditions, or a recent hospitalization and/or frequent emergency room visits. If you meet eligibility requirements, the program is part of your current Medi-Cal benefits and will assist you in; establishing a personalized care plan to assist in getting the care you need, assistance in finding doctors and scheduling appointments, and connecting you to community and social services, to assist with needs such as transportation, housing, and nutritional needs.

No, as a Medi-Cal beneficiary all services provided by the Sunrise Wellness Care program are available to you at no cost. As a Sunrise Wellness member you do not have to change your doctor, and enrollment in the Sunrise Wellness program does not affect your insurance.
